Sin embargo, considera que podemos tener agencia (libertad) en tanto, guiados por el conocimiento de las causas de nuestros afectos, actuemos en favor de nuestro conatus. Databases play a fundamental role in today’s world, being used by most companies, especially those that offer services through the Internet. Today there is a wide variety of database models, each adapted for use according to the specific requirements of each application. Traditionally, the relational models with centralized architectures have been used mostly due to their simplicity and general-purpose query language, which made relational systems suitable for almost any application. However, with the growth of the Internet in recent decades, both (...) in the number of users and in the amount of information, those centralized models began to suffer availability and scalability issues. To address those issues, the use of decentralized architectures and alternative database models began to arise, eventually replacing relational databases and centralized architectures when the requirements on availability and scalability are high. Those database models alternative to the traditional relational model are grouped under the name of NoSQL. In this article, we present a NoSQL database developed as an end of degree work, with a flexible data model based on documents and a fully decentralized architecture based on the Gossip protocol for node discovery and a distributed hash table, in particular the rendezvous hashing algorithm, used to distribute and replicate the data across all the nodes. The main goals of the system are to achieve high availability and high scalability. High availability is achieved thanks to the replication of the data, while high scalability is achieved by its decentralized architecture, which allows multiple entry points from the requests, and the data distribution, effectively increasing the database capacity by increasing the number of nodes. Background: This research explores how public awareness and attitudes toward donation and transplantation policies may contribute to Spain's success in cadaveric organ donation. Materials and Methods: A representative sample of 813 people residing in Andalusia (Southern Spain) were surveyed by telephone or via Internet between October and December 2018. Results: Most participants trust Spain's donation and transplantation system (93%) and wish to donate their organs after death (76%). Among donors, a majority have expressed their consent (59%), and few nondonors have (...) expressed their refusal (14%). Only a minority are aware of the presumed consent system in force (28%) and feel sufficiently informed regarding the requirements needed to be an organ donor (16%). Participants mainly consider that relatives should represent the deceased's preferences and be consulted when the deceased's wishes are unknown, as is the case in Spain. Conclusion: Public trust in the transplant system may contribute to Spain's high performance in organ donation. High levels of societal support toward organ donation and transplantation do not correspond with similar levels of public awareness of donation and transplantation policies in Spain. (shrink)

IntroductionThis study aimed to evaluate, in adults with mild cognitive impairment, the brain atrophy that may distinguish between three AT biomarker-based profiles, and to determine its clinical value.MethodsStructural MRI was employed to evaluate the volume and cortical thickness differences in MCI patients with different AT profiles, namely, A−T−−: normal AD biomarkers; A+T−−: AD pathologic change; and A+T++: prodromal AD. Sensitivity and specificity of these changes were also estimated.ResultsAn initial atrophy in medial temporal lobe areas was found in the A+T−− and (...) A+T++ groups, spreading toward the parietal and frontal regions in A+T++ patients. These structural changes allowed distinguishing AT profiles within the AD continuum; however, the profiles and their pattern of neurodegeneration were unsuccessful to determine the current clinical status.ConclusionsMRI is useful in the determination of the specific brain structural changes of AT profiles along the AD continuum, allowing differentiation between MCI adults with or without pathological AD biomarkers. In this paper we continue the study of the variety \ of monadic Gödel algebras. These algebras are the equivalent algebraic semantics of the S5-modal expansion of Gödel logic, which is equivalent to the one-variable monadic fragment of first-order Gödel logic. We show three families of locally finite subvarieties of \ and give their equational bases. We also introduce a topological duality for monadic Gödel algebras and, as an application of this representation theorem, we characterize congruences and give characterizations of (...) the locally finite subvarieties mentioned above by means of their dual spaces. Finally, we study some further properties of the subvariety generated by monadic Gödel chains: we present a characteristic chain for this variety, we prove that a Glivenko-type theorem holds for these algebras and we characterize free algebras over n generators. A proliferation of tests exists for the assessment of auditory-verbal memory processes. However, from a clinical practice perspective, the situation is less clear when it comes to the ready availability of reliable and valid tests for the evaluation of visual/visuo-spatial memory processes. While, at face value, there appear to be a wide range of available tests of visual/visuo-spatial memory, utilizing different types of materials and assessment strategies, a number of criticisms have been, and arguably should be, leveled at the majority (...) of these tests. The criticisms that have been directed toward what are typically considered to be visual/visuo-spatial memory tests, such as the potential for verbal mediation, over-abstraction of stimuli, the requirement of a drawing response, and the lack of sensitivity to unilateral brain lesions, mean that, in reality, the number of readily available valid tests of visual/visuo-spatial memory is, at best, limited. This article offers a critical, historical review on the existing measures and resources for the neuropsychological assessment of visual/visuo-spatial memory, and it showcases some examples of newer tests that have aimed to overcome the challenges of assessing these important aspects of memory. The article also identifies new trends and examples of how technological advances such as virtual reality may add value to overcome previous obstacles to assessment, thereby offering professionals more reliable, accurate means to evaluate visual/visuo-spatial memory in clinical practice. En el artículo se analizan los instrumentos filosóficos y literarios usados por Voltaire. En primer lugar, se analiza el lugar que ocupa la actividad literaria en la labor intelectual del autor, resaltando su inclinación a vincular la literatura con la filosofía. Después, se resalta la utilización sistemática de la fábula y la parábola en el desarrollo de la historia. En tercer lugar, se ilustra la forma en la que ciertos acontecimientos vitales determinan la evolución de la disputa entre optimismo y (...) pesimismo. Finalmente, se resalta la habilidad literaria de Voltaire al desarrollar la disputa en temas como la filantropía, la guerra, el amor, los infortunios, el suicidio, el mal y la melancolía. Se defiende la tesis de que para mantener viva la disputa el autor sacrificó la verosimilitud, ejerciendo una actitud filosófica-literaria de apertura, es decir, una concepción de reflexión y continua polémica. In this paper we study the subdirectly irreducible algebras in the variety of pseudocomplemented De Morgan algebras by means of their De Morgan p‐spaces. We introduce the notion of the body of an algebra and determine when is subdirectly irreducible. As a consequence of this, in the case of pseudocomplemented Kleene algebras, two special subvarieties arise naturally, for which we give explicit identities that characterise them. We also introduce a subvariety of, namely the variety of bundle pseudocomplemented Kleene algebras, fully (...) describe its subvariety lattice and find explicit equational bases for each subvariety. In addition, we study the subvariety of generated by the simple members of, determine the structure of the free algebra over a finite set in this variety and their finite weakly projective algebras. El desarrollo de la ciencia y la tecnología y de las competencias técnicas ha impulsado el desarrollo de las sociedad haciéndolas más complejas, causando que las aptitudes emocionales se conviertan en factores de éxito, ya que afectan el desempeño humano en todos los escenarios donde se desenvuelve, no obstante a pesar de las investigaciones en este área, actualmente no se cuenta con modelos que relacionen dichas aptitudes con el rendimiento organizacional, lo cual dio origen a esta investigación que busca determinar (...) el impacto de dichas competencias en la ejecución de proyectos en la empresa ENGISERV VENEZUELA C.A. Para realizar el estudio la metodología usada considero que se trata de una investigación de campo, no experimental y transversal. Since then, objections against the diagnosis of brain death from family members of those diagnosed as dead-have been increasing and are causing some countries to take novel steps to accommodate people’s beliefs and preferences in the determination of death. This, coupled with criticism by some academics of the brain death criterion, raises some questions about the issues surrounding the determination of death. A cognitive assessment strategy that is not limited to examining a set of summary test scores may be more helpful for early detection of emergent illness such as Alzheimer’s disease (AD) and may permit a better understanding of cognitive functions and dysfunctions in those with AD and other dementia disorders. A revisit of the work already undertaken by Kaplan and colleagues using the Boston Process-Approach provides a solid basis for identifying new opportunities to capture data on neurocognitive processes, test-taking strategies (...) and response styles. Thus, this critical review will combine traditional process-based assessment strategies with support provided or offered by newer technologies that have the potential to add value to data collection and interpretation. There is now considerable interest in neuropsychological test administration using computer/digital technology, both in research and in clinical settings. To add value, any computerized version of an existing cognitive test should respect the administration procedure for which normative data were obtained, should be time-saving in terms of scoring and interpretation, and should, we argue, facilitate gathering information about the processes and strategies followed in test completion. This article will offer an overview of the steps needed when implementing computerization of neuropsychological tests using a Process-Based Approach (PBA) to these technology-based adaptations and will discuss further developments in this area by linking it to future technological developments that may be possible in the area of neuropsychological assessment. Additionally, an overview of neuropsychological tests that may benefit from computerization will be presented, together with suggestions on the specific processes, strategies and features that may be captured with the aid of such computerization. Finally, hypotheses on how virtual reality could be an asset for the future of the PBA to neuropsychological assessment will also be discussed. (shrink)

Requiring family authorization for apnea testing subtracts health professionals control over death determination, a procedure that has traditionally been considered a matter of clinical expertise alone. In this commentary, we first provide evidence showing that health professionals’ (HPs) disposition to act on death determination without family’s prior consent could be much lower than that referred to by Berkowitz and Garrett (2020). We hypothesize that HPs may have reservations about their own expertise as regards death, and may thus hesitate to impose (...) their views on patients’ families. We then address the theoretical question of clinical expertise in death determination by distinguishing judgments about facts (e.g., the presence or absence of spontaneous breathing) from interpretations given of these facts (i.e., their meaning for the vital status of an individual). We argue that, while clinicians may claim some expert authority on the former, they hold no particular authority on the latter. (shrink)

Los cuidados paliativos buscan ofrecer una atención holística para brindar una mejor calidad de vida a los enfermos, especialmente cuando se aproximan al fin de su existencia. Profundizar en la dimensión espiritual de la persona humana resulta atractivo puesto que ofrece respuestas al sentido de la vida. Se ha constatado que la atención espiritual aporta grandes beneficios para el paciente. Es por eso que en este trabajo hemos querido profundizar en la importancia de la dimensión espiritual en los cuidados paliativos; (...) se destaca la actitud de la escucha como pieza clave de dicha atención espiritual, lo que evidencia que el burnout de los médicos es el principal obstáculo para potenciar la escucha como una adecuada disposición que resulte consoladora y sanadora. La filosofía de la tecnología es una rama reciente de la filosofía en general. Puede aportar herramientas para problemas derivados de aplicaciones tecnológicas recientes, como la telemedicina. Un problema señalado reiteradamente por la bioética lo constituye la privacidad en general, y las modificaciones que pueden introducirse por el cada vez mayor uso de las tecnologías. Comentando propuestas desde la Asociación Médica Mundial y datos contemporáneo derivados de la pandemia de COVID-19, puede reconocerse que la privacidad es una exigencia ética que (...) requiere regularse a través de su implementación política para su adecuada protección legal. El artículo plantea condiciones clínico-epidemiológicas peculiares cuando la humanidad se enfrente a una zoonosis emergente. El ejemplo que groseramente ha golpeado a la humanidad es la pandemia de Covid-19, enfermedad causada por el coronavirus SARS-CoV-2. Si bien la historia de la humanidad da cuenta de múltiples pandemias ocasionadas por varios agentes infectocontagiosos, es cierto que en el primer momento de afrontar una problemática global de esta naturaleza lo que prima es la incertidumbre. Ante ella, existe la obligación moral de proveer (...) los mejores servicios de salud que sea posible; a la par, debe realizarse una investigación para encontrar medidas farmacéuticas específicas que contengan y mitiguen el daño poblacional (medicamentos específicos y/o vacunas). Mientras tanto, sabiendo que en el caso de la actual pandemia de Covid-19 la principal y más eficiente vía de transmisión es la propagación por gotas de saliva, el distanciamiento físico es crucial. Una consulta médica involucra hablar en el interrogatorio y una proximidad física durante la exploración. Existen datos que indican que la telemedicina tiene un alto grado de recomendación para la protección de la salud, tanto de profesionales como de pacientes. Se recuerda que una declaración de la Asociación Médica Mundial ha tratado el tema de la telemedicina; con su exposición, se indican alcances, limitaciones y proyecciones en el futuro. (shrink)

Altruism and solidarity are concepts that are closely related to organ donation for transplantation. On the one hand, they are typically used for encouraging people to donate. On the other hand, they also underpin the regulations in force in each country to different extents. They are often used indistinctly and equivocally, despite the different ethical implications of each concept. This paper aims to clarify to what extent we can speak of altruism and solidarity in the predominant models of organ donation. (...) It also raises the ethical question of whether these categories are adequate as a basis for such models, bearing in mind that organs are a scarce resource and that a shortage of them may mean that fewer lives are saved or improved. Cybersecurity aggregates are numerical data obtained by aggregation on features along a database of cybersecurity reports. These aggregates are obtained by integration of time-stamped tables using some recent results of non-standard calculus. Time-series of aggregates are shown to contain relevant information about the concrete system dealt with. Trend time series is also forecasted using known data-driven methods. Although absolute forecasting of trend time series is not obtained, a directional forecasting of trend time series is achieved thence validated by means of (...) a rolling cross validation scheme on a public database of Scareware reports. Dentro de este contexto, se realiza una discusión en torno al Estado, la construcción de la nación y la utilización de la escuela como instrumento disciplinador de la sociedad. Asimismo, se analiza la experiencia histórica de la escuela en el sur peruano, como parte de un proceso que antecede a la instauración del modelo educativo nacional chileno, relevando la (...) praxis política que las propias comunidades asumen en torno diversos petitorios por la presencia de profesores y de establecimientos educacionales en los sectores cordilleranos andinos. Context: The emergency situation caused by coronavirus disease 2019 (COVID-19) has affected different facets of society. Although much of the attention is focused on the health sector, other sectors such as education have also experienced profound transformations and impacts. This sector is usually highly affected by psychosocial risks, and this could be aggravated during the current health emergency. Psychosocial risks may cause health problems, lack of motivation, and a decrease of effectiveness at work, which in turn affect the quality of (...) teaching. Despite their importance, there are hardly any studies that analyze psychosocial risks of non-university teachers during a health emergency such as that caused by COVID-19. Objectives The aim of this study was to analyze the perception of COVID-19 and the psychosocial risks of non-university teachers comparing Spain and Mexico during the state of alarm caused by COVID-19. Methods Data were collected from 421 non-university teachers (80.2% women; 56.3% from Mexico, 43.7% from Spain) aged 24–60 ( M = 39.32, SD = 10.21) via a self-completed questionnaire during the pandemic from March to April 2020. Results Data analysis suggests that inequity is the most important risk, followed by work overload. Teachers appear to be moderately satisfied with the information on COVID-19 and the measures taken, while their satisfaction with the available resources is lower. When comparing the two countries, significant differences can be observed in every risk considered except for social support, with lower levels in Mexican teachers compared to Spanish ones. In the case of the perception of COVID-19 and its impact, the perception in general of levels of information, measures, and resources is better among Mexican teachers than among Spanish ones, who present higher scores of the impact of the health emergency. Conclusion The results underline the importance of the professional’s perception of resources during a health emergency, which could prevent to some extent burnout and possible alterations associated with it. The measures taken by the responsible entities and the provision of information do affect teachers not only directly but also indirectly by making them more vulnerable to psychosocial risks that could affect their health and professional performance, thus affecting students as well. An algebraically expandable (AE) class is a class of algebraic structures axiomatizable by sentences of the form $\forall \exists! \mathop{\boldsymbol {\bigwedge }}\limits p = q$. For a logic L algebraized by a quasivariety $\mathcal {Q}$ we show that the AE-subclasses of $\mathcal {Q}$ correspond to certain natural expansions of L, which we call algebraic expansions. These turn out to be a special case of the expansions by implicit connectives studied by X. Caicedo. We proceed to characterize all the AE-subclasses of (...) abelian $\ell $ -groups and perfect MV-algebras, thus fully describing the algebraic expansions of their associated logics. (shrink)

El uso de la Inteligencia artificial (IA) en la Medicina de laboratorio (ML) ha provocado un salto cualitativo en el diagnóstico de enfermedades que aquejan al ser humano. El desarrollo de robots para la medición, cálculo y predicción ha aumentado la confiabilidad, validez y reproducibilidad de las pruebas diagnósticas con IA, induciendo a una fácil elección de dicha tecnología en el laboratorio clínico. Sin embargo, la IA en la ML entraña una serie de reflexiones éticas que deben considerarse. La incipiente (...) tecnología en desarrollo, la presencia de sesgos cognitivos en los algoritmos y datos, la incertidumbre del funcionamiento del robot, las limitaciones tecnológicas, la amenaza a la privacidad y la ausencia de un marco legal abren conflictos éticos que laceran la equidad, la seguridad y la autonomía del hombre. El imperativo tecnológico de la IA en la ML no debe superar la responsabilidad, ni atentar contra la dignidad de la persona. (shrink)

To understand Lacan’s thinking process on vision, the entirety of his teaching must be taken into consideration. Until the 60s, the visual field is the imaginary, the constitutive principle of reality in its phenomenal giving to the experience of a subject. This register is the opposite of the field of the word with the L schema and, subsequently, as subordinated to the symbolic system according to the model of the optical schema of the inverted flower vase of Bouasse. It is (...) only with the 1964 seminar that Lacan makes a daring turnaround through which the visual becomes a sign of the emergence of a real that is irreducible to both reality and the mediation of the subject of knowledge. The split that separates reality and the real is reproduced in Lacan within the visual field, which is, on the one hand, the cardinal principle of the consistency of the experience of reality, and on the other, it is an element of irreducibility to reality. This produces a cascade of consequences: first of all, the modification of the presentation of the mirror stage. Unlike the voice, which through prosody, tone, and volume, finds some strips with which anchor itself imaginatively to reality, the gaze, invisible and elusive, escapes the imaginary grasp. Captured in myths, it reveals its power and ability to annihilate—as in the myth of Medusa’s gaze—or to make people fall in love but only with a narcissistic love that leads inexorably to death as in the myth of Narcissus. The gaze is elusive because the subject is dependent on it in the field of desire. Like the voice, it is about the desire on which the subject is supported; it is one of the objects on which the phantom depends. In our opinion, thanks to this characteristic, the gaze object can make remote psychoanalytic treatment possible through easily accessible videoconferencing tools and, at the same time, create new conditions within it that should be carefully evaluated to understand its implications in the session itself. Dos místicos relativamente contemporáneos: Etty Hillesum y Benjamín González Buelta; dos estilos, dos contextos y una coincidencia establecida desde sus experiencias espirituales, en profunda conexión con la realidad, en medio de la vida y sus desafíos. Hillesum no habla propiamente de la experiencia mística, sin embargo, su proceso personal existencial en el duro contexto de guerra que le tocó vivir, la condujo a descubrirse profundamente habitada. Su experiencia de Dios le hizo posible comprender y gestionar el dolor en medio de (...) la guerra, así como la necesidad de ‘hacerse bálsamo sobre las heridas’ de sus hermanos judíos. Etty es una mística de ojos abiertos. Benjamín González Buelta ha profundizado en la comprensión de la mística, describiéndola y analizándola desde la experiencia de Dios en el contexto latinoamericano. Su reflexión permite hoy leer a Etty Hillesum desde la perspectiva del místico de ojos abiertos. (shrink)
